Insurance Policy

We are contracted with most major insurers and are continuing to add more. Please contact your insurance company to verify if we are a contracted provider with your insurance. If you are a Medicaid patient, please contact our individual Care Center as we may or may not be accepting new Medicaid patients at that time.

We advise that all patients verify that your Care Center is an in-network provider for your insurance. Also, check which lab your insurance company is contracted with. You are responsible for any fees that are not paid by your insurance carrier.

Newborn Notice: Prior to your first appointment, please be sure that you have contacted your insurance company to add your new baby to the insurance policy within the first 30 days of life. Also make sure to bring your insurance card with you to your first appointment, so we have everything needed to submit your baby’s claims after you have added your baby to your plan. If your claim is denied, you will be responsible for payment.

Due to the lack of compliance by cost share plans, our Care Centers will no longer be accepting cost share plans. Cost share plans are not actual insurance plans. They are group plans where members pool funds together to help cover those greatest in need. The cost share plan administrator is responsible for managing the funds that the members provided. Cost share plans do not have contracts as they are unable to guarantee payments for the services provided to you.
